WILC/PILS are consumer controlled nonresidential centers for independent living, which provides information and advocacy to persons with disabilities. In addition, WILC/PILS operates a Regional Resources Development Services Program (seven counties). The goal of this Program is to develop home and community-based services and supports needed by individuals and family members who experience a disability.
POSITION: Waiver/Lead Nurse Evaluator (NE)
HOURS: Full time, 35 hours per week, with benefits
RESPONSIBILITIES:
- Conduct Eligibility Assessments (UAS);
- Assist with applicant screens for medically complicated individuals
- Assist with the review of service plans submitted for individuals that are medically complicated;
- Review, track and monitor, UAS reports, medically complicated Serious Reportable Incidents and Recordable Incidents for the LHV;
- Supervise Nursing Evaluator (NE)Team
- Assign and Track NE UAS completion,
- Oversee submission of NE paperwork. reports, vouchers, travel logs etc.
- Provides information and referral to services available to individuals with disabilities in the Lower Hudson Valley;
- Assist with preparation and submission of reports and tracking forms;
- Maintain files on all waiver participants;
- Provide technical support by attending team meetings, discussing concerns and recommendations with providers;
- Assist with developing resources and providing information to build the Regions capacity to serve the waiver participants;
- Assist in arranging the required trainings/workshops for Hudson Valley waiver providers and community;
- Attend required meetings and conferences for DOH waiver and;
- Conduct any other duty as assigned by Lead RRDS/SR RRDS that will promote and enhance the development of the waiver in the Hudson Valley Region;
QUALIFICATIONS:
- New York state licensed registered professional nurse (RN)
- Two (2) years’ experience working with individuals with disabilities and/or seniors in a community setting
- Experience supervising professional staff
- Excellent clinical assessment skills
- Demonstrated abilities in written and oral communication skills
- Ability to use word processing computer programs
- Well Organized and able to work independently
- Reliable transportation, valid driver’s license and insurance coverage
